We are always asking ourselves several questions to guide us in what we decide to do and how we plan to carry out an initiative in any of the model levels indicated above: 1. Do we have a significant community health problem in terms of severity and extent of disease burden? 2. Do we have a proven best practice intervention that will address the problem? 3. Do we have the resources (e.g., staff, space, funding) to implement the intervention? 4. Do we have other community partners in coalitions, councils or contracts that may collaborate with us in a given initiative? 5. Do we need to have formal agreements or legal documents to implement the chosen initiative?
