In 2022 Senator Heinrich secured a Congressionally Directed Spending grant to help FYI+ began their development as New Mexico's first Certified Community Behavioral Health Clinic. New Mexico has since been added to the U.S. Department of Health and Human Services’ expansion of the CCBHC Medicaid Demonstration Program, allowing for more stable funding for access to services, according to the department.
Amador Health Center (AHC), a federally qualified health center (FQHC), is now formally partnering with Families & Youth Innovations Plus (FYI+) as a DCO (Designated Collaborating Organization) in their Certified Community Behavioral Health Center (CCBHC). Both organizations are community non-profits that accept Medicaid and most private insurance and have a sliding fee discount program. Agreements for Co-Location, Interagency Services, and Evaluation were signed in 2024.
The CCBHC project implementation will be carried out in two phases as noted here. AHC Tier 1 On/around October 1, 2024; establish Outreach Office for Recovery Support Services (RSS) at FYI+; harm reduction supplies with syringes & safer smoking kits and education, rapid testing for infectious diseases, acudetox. Medication assisted treatment (MAT) with buprenorphine/naltrexone on site, via video conferencing, home visits, and at community partner prescribers. Medication reminders, delivery, observation. Accompany client to appointments. Case Management and Care Management, Coordination, Coaching, and Contingency Management. Peer and community support, income and housing support. Individual counseling and instruction. Groups, meetings and classes.
AHC Tier 2 Estimated implementation in 1st Quarter of CY 2025; Primary Care Clinic (preventive, acute, chronic care; Gender Affirming Care) onsite 3 days a week or with Mobile Clinic at Solano facility and Psychiatric Medication Management via telehealth (PMHNP, RxP).
We anticipate that this collaboration will have a win-win-win result-- A. FYI+ will have integrated, comprehensive medical/behavioral services on site; B. AHC will have new access point outside the MVCH campus; C. Las Cruces residents will have available a true collaborative care model (CoCM) provided by FYI+ and AHC together; thus reducing fragmentation, gaps, and duplication of services while promoting innovation in service delivery.
